1. Dealing with surprises in the line organization

In a traditional hierarchy, power and decision-making flow from the top down. Tasks are allocated across departments, and a single manager bears overall responsibility. This model works well in stable environments, but quickly reaches its limits when faced with unexpected changes. Decisions become bogged down with a handful of managers, problems are passed up through several levels of the hierarchy, and valuable time is lost.
